Paid Time Off (PTO) Requests
PTO requests must be submitted via this Time Off Request Form no less than one week in advance. Should you have any questions or mitigating circumstances, please see your Manager. The Handoff Form should also be filled out to help distribute your responsibilities while you’re away. Once approved you'll be emailed a Google Calendar event. PTO Accrual can be found in ADP's Employee Access, listed below in the Payroll section. PTO may not be rolled-over to the next year, cannot be cashed out and expires 12/31 at 11:59pm
Sick Leave Requests
Sick Leave requests must be for a legitimate ailment or doctor’s appointment. SL requests must be submitted via this Time Off Request Form no less than one day in advance. SL Accrual can be found in ADP's Employee Access, listed below in the Payroll section.
Hourly employees can clock in and out of their shifts via Homebase using their respective PIN numbers on the Assembly Room computer. For issues with missed or incorrect hours please email email@example.com.
Well Made uses a software called ADP to manage and process payroll. Payroll is processed on the 1st and 15th of each month. ADP will allow you to access Pay Statements, Direct Deposit information, tax forms as well as PTO and Sick Leave accruals.
During your employee onboarding period you will be emailed a link to create an account with ADP's Employee Access. You may use one of Well Made's computers to access my.adp.com, or download the ADP Mobile Solutions app on your phone.

In the case that you need reimbursement for company-based expenses, an Expense Report must be submitted via this Expense Report Form and receipts must be submitted. A physical check will be issued with 2-3 business days.
Well Made is proud to offer its Full Time employees health insurance options and a contribution of $100 (salaried) or $50 (hourly) per pay period, with the employee covering the rest. Should you be new to Well Made, below are our plan options. Please choose one and schedule a time with Brandon to fill out the appropriate paperwork. Full Time employees are eligible at the first open enrollment period after 30 days from their hire date.
